I'm trying to find the best way to document a child that was adopted by his birth mothers' 2nd husband. Do I enter him twice - once with his birth name to his birth parents and again with his adopted name under his birth mother and her new husband? Please advise. For example, we can call his birth name as John Jones and his adopted last name as Anderson.
